Östrojen kullanımı konmamış anormal vajinal kanaması olan hastalarda kullanılmamalıdır. (Birlikte progesteron tedavisi olmadan yani östrojen) uzun süreli (>1 yıl), rakipsiz östrojen kullanımı rahim kanseri önemli doza bağlı risk ile ilişkili olmuştur. Riskini önemli ölçüde bir kez, ek telafi edilebilir ama tamamen kaldırılmış olabilir. Östrojen tedavisine başlamadan önce uygun tanı testleri anormal vajinal kanaması olan hastalarda endometriyal kanser ekarte etmek için yapılmalıdır. Eğer Tekrarlayan veya sürekli kanama östrojen tedavisi sırasında gelişen aynısı geçerlidir.
