Östrojen genellikle plazma lipitleri üzerine olumlu etkileri, HDL ve total kolesterol ve LDL düşüşler artışlar dahil olsalar da, özellikle yüksek dozlarda kullanıldığında trigliserid düzeylerinde anlamlı yükselme ile ilişkili olmuştur. Şiddetli hiperlipidemi bazen pankreatite neden olduğu bilinmektedir. Hiperlipidemi olan hastalarda buna göre yağ kaydettikleri östrojen tedavisi sırasında yakından izlenmesi ve ayarlamalar-rejimi düşürmek gerekebilir.
