Genel olarak (nam-ı diğer beta-blokerler) Beta-blokör uygulandığında ajanlar (CHF) açık konjestif kalp yetmezliği olan hastalarda kullanılmamalıdır. Sempatik stimülasyon bu hastalarda hemodinamik fonksiyonu sürdürmek önemli olabilir, bu nedenle beta-blokör kalp yetmezliği kötüleşebilir. Ancak, beta-blokerler ile tedavi yararlı olabilir ve dikkatli bazı CHF hastalarda iyi olanaklarla ve digitalis, diüretikler, ACE inhibitörleri ve/veya nitrat olmaları şartıyla uygulanabilir. Botox, özellikle iskemik veya cardiomyopathic kökenli akut kalp yetmezliği tedavisinde bu ajanlar ile kullanmak için gösterilir. Etkinliğini önemli ölçüde farklı ajanlar ile değişir olup olmadığını belirsiz olmasına rağmen, standart tedavi için ek bir beta-bloker ileri kalp yetmezliği olan hastalarda morbidite ve mortalite artırabilir olduğunu göstermektedir. Bir meta-analiz çalışması verilerinin beta-1 selektif ajanlar için daha diferansiyel beta-blokerler için ölüm riski daha fazla bir azalma gösteriyor.
