Anjiyotensin II reseptör (AR) antagonist renal fonksiyon sistemi klinik ACE inhibitörleri renin aktivitesine bağlı olan hastalarda böbrek yetmezliği neden olabilir. Buna ek olarak, semptomatik hipotansiyon, böbrek ve miyokardial perfüzyon tehlikeye atabilecek duyarlı bireyler oluşabilir. (USD) şiddetli konjestif kalp yetmezliği olan hastalarda, AR antagonist ile tedavi oligüri ve/veya ilerleyici azotemi ve nadiren, böbrek yetmezliği, miyokardiyal iskemi ve ölüm ile ilişkilendirilmiştir. AR antagonist ile tedavi özellikle hacim ve/veya sodyum azalması ile birlikte dikkatli biçimde şiddetli kalp yetersizliği olan hastalarda başlatılan olmalıdır. Böbrek fonksiyon düşüş yaşayan hastalarda, AR antagonist tedavi sırasında genellikle kalp yetmezliği ve böbrek hasarı semptomatik iyileşme iyi tolere sağlanan gerekli değildir. Geçici hipotansiyon de tedavi genellikle kan basıncı stabilize sonra zorlanmadan iade edilecektir beri AR antagonist ile tedavi daha orantılı değildir.
